The Oregon Department of Human Services(DHS), Office of Licensing and Regulatory Oversight, is seeking TWO experienced and professional, permanent full time, Client Care Surveyors to join our team in Salem. These positions are represented by a union.


What you will be doing:

The purpose of the Client Care Surveyor position in the I/DD Licensing Team is to inspect and review the quality and adequacy of care and services received in a variety of community developmental disability service settings. This requires knowledge of multiple administrative rules governing these settings including but not limited to Medicaid Agency Certification and Endorsement, 24-Hour Residential Programs for Children and Adults, Employment , Supported Living Programs., Support Services Brokerages,Professional Behavior Services and Community Living Supports. Such reviews must be accomplished within mandated time frames. These surveys determine the programs compliance with federal and/or state regulations, and ensure that individuals receiving services are healthy and safe and the programs deliver the service outcomes within the scope of individual planning. Surveyors provide professional evaluations of findings during the surveys and make determinations regarding licensing compliance. Work is performed independently, as well as in a team setting, in accordance with regulations and rules, professional standards of practice, and DHS and Team procedures.

WORKING CONDITIONS:

Position requires frequent travel to Counties throughout Oregon. Overnight travel in order to complete projects may be necessary, extended stay up to two consecutive weeks should be expected at least once a year. Adjustments to work week to accommodate evening meetings or travel are necessary. Position requires excellent oral and written communication skills, ability to report on findings, and provide Technical Assistance. Requires use of a desktop computer and an electronic calendar. This position is a mandatory reporter of abuse and neglect of children, seniors and other adults with disabilities (mentally ill and developmentally disabled).





Duties & Responsibilities



Survey and Inspection: Works with a team, either as a team member or a team leader. Surveyors work both independently for some tasks and in a consensus process for others. Conduct thorough on-site reviews of programs to monitor and ensure compliance of Department certified agencies/endorsements, 24-hour residential, employment services, supported living programs, and support services programs with applicable Oregon Administrative Rules (OAR). Such reviews shall at a minimum be conducted in accordance with the program's licensing or certification cycle, upon receipt of any complaints regarding compliance with such rules, or at the request of the manager for problem programs. This includes:

a) On-site review of program and individual records regarding rights, health, safety and other minimum requirements set in administrative rule or statute, as well as observation and interviews conducted at the site of staff and individuals available.

b) Reviewing the program history, serious event reports, complaint investigations, and Therap or any other electronic records that are available.

c) Compiling findings with the survey team in a consensus process, to determine levels of compliance or non-compliance and verbally reviewing findings with facility administration and staff.

d) When necessary acts as a witness in contested case hearing providing evidence regarding survey findings as part of the Department's case regarding regulatory actions.







Qualifications, Required & Requested Skills

MINIMUM QUALIFICATIONS:


Three years of experience in areas related to nursing, social work, developmental disabilities, dietetics/nutrition, or sanitation; AND


One of the following:


A Bachelor's degree in a health or human services field;


OR


A license as a Registered Nurse.


LPN certification and/or experience will not meet the minimum qualifications.



REQUESTED SKILLS:





+ Experience working in the health care field, including assisted living, residential care, or nursing facilities.

+ Experience applying Federal and State laws, rules, standards and guidelines about the determination of level and quality of care for residents requiring long term direct care.

+ Skill in exercising independent judgment and decision making in the application of laws, rules, policies, and/or procedures.

+ Skill in communicating verbally with a variety of medical professionals, care provider management and staff, resident relatives, and personnel or other agencies, sometimes under tense or hostile conditions.

+ Skill in putting work in priority order and managing work time and efforts to accomplish both ongoing and emergent duties.

+ Skill in making accurate verbal and written reports.

+ Ability to demonstrate flexibility relating to emergent changes in schedule and team assignment.

+ Ability to use a computer to input, retrieve information and generate information and/or reports.
